1) Taxi: 40k for foreigners, 30k for "locals" or assimilated. Ojek: 10 might be enough, but I'd leave 15k because the driver pays some k's for entering the parking.
2) I don't know the average, but I know the variance is high. It depends on the level of the place, and it's always negotiable. At Formosa, massage is 70k/h (yes!! less than 10SGD), but girls dare to ask 300k for HJ.
3) Some nice guys responded me when I asked the same question. Most times, I ate it at Nagoya Hill Street Food. I guess it is acceptable also for experts (but I am not)
